Manchester United are seeking to complete a first league-double over their city neighbours since 2008/09; Manchester City have won six of their last nine outings at Old Trafford.
Neither of these two fierce rivals are enjoying seasons to remember, but there's always so much on the line when derby day rolls around in Manchester.
Manchester City moved three points closer to Champions League qualification on Wednedsay, swatting aside Leicester 2-0 at the Etihad, thanks to goals from Jack Grealish and then Omar Marmoush. That was just the Sky Blues' third victory in seven Premier League games, but they do remain fifth, thereby occupying the final Champions League position.
Pep Guardiola's team could also end this season with silverware, having reached a seventh successive FA Cup semi-final by ousting Bournemouth last Sunday. Back in the league, the Citizens need points if they're to remain in the top five, targeting a memorable derby day victory here.
As for Manchester United, well they lost again on Tuesday, beaten 1-0 by Nottingham Forest, with, of course, Anthony Elanga the match-winner against his former employers. That is the 13th time the Red Devils have tasted defeat in the league this season, only enduring more than that once since 1990, and that was last season.
Rúben Amorim's team therefore remain rooted in the bottom half of the table, and things don't get any easier, with this derby followed by a trip to Newcastle. Before then, Man United will also take on Olympique Lyonnais in the Europa League quarter-finals, with the first leg coming up in France on Thursday. That competition is the only way Man United can salvage this season, although a derby victory, as they enjoyed in dramatic fashion at the Etihad in December, will certainly help.
Lisandro Martínez, Jonny Evans, Luke Shaw, Ayden Heavan, Kobbie Mainoo and Amad do remain sidelined long-term, but Rúben Amorim's injury concerns are easing. Both Harry Maguire and Mason Mount returned off the bench on Tuesday, while fit-again Leny Yoro also started.
Despite having ended his goal draught during Man United's previous outing at Leicester, Rasmus Højlund was only a substitute against Nottingham Forest, so could he start this time?
Manchester United predicted lineup vs Manchester City: (3-4-3): Onana; Yoro, de Ligt, Mazraoui; Dalot, Ugarte, Casemiro, Dorgu; Garnacho, Fernandes, Højlund.
Bruno Fernandes has 16 goals to his name this season, one of which was a penalty at the Etihad that sparked the comeback in December, so can he add to his four goal against Man City?
John Stones, Manuel Akanji, Nathan Aké and Rodri remain sidelined, while Erling Braut Håland could miss the remainder of the Premier League campaign, after suffering a knee injury at Dean Court. In better news, Oscar Bobb came off the bench on Wednesday, his first appearance since the Community Shield due to a broken leg, while Vitor Reis also made his debut as a substitute.
Nico O'Reilly changed the game off the bench against Bournemouth, therbey earning a start at left-back against Leicester, so will he keep his place? Lastly, after ending his 473-day wait for a Premier League goal, will Jack Grealish keep his place, or will Phil Foden or Kevin De Bruyne or both return?
Kevin De Bruyne announced on Friday he'll leave Man City at the end of the season, meaning this'll be his 21st and final Manchester derby, seeking to add to his three goals against Man United.
We're forecasting a major Manchester derby upset, backing Man United to claim a surprise win over Man City. Are we confident about that? Not especially: Manchester United 2-1 Manchester City.
